I remember when I ordered stuff on Amazon and it just came; never had delivery issues. Now like 10% have issues. Doesn't seem that high, unless you order about 10 things a week and then you're talking a problem a week.

Generally it's an issue before the delivery part. Often it starts from the wrong hub. If I see "Out for Delivery" but it lists a neighboring city in the metro I know it's not coming. Sometimes it will never get picked up. Others it just drops off during the process.
I'm surprised Amazon would still have those issues at their maturity level, I guess we've fared pretty well in my area, only a few times has something not shown up
